Forms & Payment

If you're accepted into the ASI Youth for Jesus program, you must complete our medical release form and letter of agreement. The medical release form must be notarized. Please mail those to the address specified in the forms. Also you will need to include your $650 program fee.  The address that all forms and payment should be sent to is:

Application Fee $35.00 USD Participant Fee $650.00 USD

The application fee needs to be submitted when you fill out your application.

Applications will be reviewed and notices sent out in February of acceptance/waiting list/not accepted into the program.

The participation fee should only be submitted once you have been accepted into the program.
